Veterinary Referral form

We work as part of a vet-led team and see all behaviour cases on veterinary referral (as per the code of conduct for ASAB Acc, FABC and ABTC). We therefore ask for the referral form below to be completed for behaviour referrals, and for a copy of the patients medical history (including test results) to be uploaded.

Post consultation we will liaise with you and send a copy of the behaviour report. If there are any questions at any point, please do not hesitate to contact us.
